A bug in the processing of larger data batches for report generation caused duplicate records to appear in the User Activity report. The fix for this was released to Production on March 19th - the duplicate records no longer appear.

An issue prevented the option to skip a non-mandatory free text question from appearing for users who had already begun text entry. This has been resolved with a 'Clear & Skip' button.

The character limit for the Notes section has been increased from 2000 characters to 5000 characters.
